Please mail this completed registration form and the entry fee of $250 (payable to Wappinger United Soccer Club) to: Alex Flores/WUSC, 21 Brentwood Drive, Poughkeepsie, NY 12603. The fee is non-refundable upon acceptance to the tournament unless a substitute team is able to fill the spot.

Each team participating in the tournament does so at its own risk and agrees to accept responsibility for any injuries. Neither Wappinger United Soccer Club nor Vassar College will accept any responsibility or liability for any injury received during the tournament.
